2022 - Homework Centre Student Registration Form

We know that finding the time to get homework done can be a challenge for busy families. To help ease the burden, students can now complete their homework in a supervised learning environment before they go home from school.

Our school’s Homework Centre will be supervised by teacher aides in the Resource Centre and will be open starting Tuesday 08 February:

Tuesday from 3.00 pm. to 3.50 pm.
Wednesday from 3.00 pm. to 3.50 pm. and
Thursday from 3.00 pm. to 3.50 pm.

The sessions are free, and a healthy snack will be provided.

Completion of this registration form does not confirm a position.

Activity Risks Insurance - The activity outlined above carries an inherent risk of physical injury occurring. Please note that the Department of Education and Training does not have personal accident insurance cover for students. If your child is injured as a result of an accident or incident, all costs associated with the injury, including medical costs are the responsibility of the parent/carer. Some incidental medical costs may be covered by Medicare. If you have private health insurance, some costs may be also be covered by your provider. Any other costs must be covered by parents/carers. It is up to all parents/carers to decide what types and what level of private insurance they wish to arrange to cover their child. Behaviour - I understand that if my child demonstrates behaviour that is not responsible, they are a danger to themselves, other participants and the success of the program or my child refuses to abide by set responsibilities I shall be contacted. I understand my child may need to return to school or home and I am responsible for their transportation from the camp/excursion location. *

I understand that if my child is at all unwell, they cannot attend. If my child becomes unwell a parent/carer or emergency contact will collect my child promptly. *

- I understand that teacher aides are not to instruct my child or provide personalised tuition. Teacher aides will supervise my child during the session.
- I confirm that my child will be collected from the Homework Centre at the time that the Homework Centre session finishes (e.g. 3.50 pm.) by myself or an approved adult emergency contact.
- I confirm that if my child is booked to attend the Homework Centre but will not attend on a particular day, I will contact the school administration staff to advise of this.
- I understand that poor attendance can result in our position being offered to another student on the waiting list.
